If you are not going to design your own forms, you may use the format(s) pre-defined in SUMMIT. These pre-printed forms have a standard layout with options for:
To order invoices, A/R statements and/or checks on pre-printed forms contact VERIBEST at 1756 38th Street, Boulder, CO 80301-9006. Ask for Kim Kendall (303)443-4570 or (800)422-7004. PRINTING: Laser - cut sheet printing is the least expensive way to go. Pre-printed Invoice and Statement forms start at a minimum order of 1,000 with a cost of $80.00 per order. Pre-printed check forms also start at a minimum order of 1,000 with a cost of $99.00 per order. Checks use magnetic ink and have an endorsement pre-printed, which brings the pricing up a little. Keep in mind, the more you order the less per thousand price you’ll pay. It pays to think ahead about usage. Tractor feed - impact printing is more expensive than laser printing. The minimum order is 1,000 and the price ranges from $150.00 - $210.00 depending on size, type of form and customization. Turn around time (including shipping days) via UPS is 3 weeks for all orders. Note that A/P and A/R checks can be either generic (for all purpose use - even payroll) or an exclusive A/P check, which has the stub column header pre-printed for you. If you use a separate bank account for A/P and payroll you will want to order the exclusive pre-printed A/P check. Remember, you may need to order W-2’s using the standard single width government approved format. You can either order from VERIBEST or an office supply store. Determine how many copies you will need and what the government requires prior to ordering. VERIBEST and SUMMIT have laser and tractor feed samples for your review if needed. Alternatives to ordering pre-printed statements: use blank paper and SUMMIT’S preset customization in the utilities menu or define them in the appropriate customization menu. Another alternative is to use a local printer. You may even be able to set up trade. VERIBEST can send base artwork for $20.00-$30.00 in two (2) to three (3) day's time.
